Qualifications

• Bachelor of Nursing preferred with minimum of one year of experience.
• RN staff hired on or after July 1, 2013 with an Associate or Diploma degree in nursing must enroll in an accredited BSN program within two years and obtain BSN within four years.
• Prior leadership experience preferred.
• One year of case management and/or utilization review work experience preferred.
• Case Management certification preferred.

Responsibilities

• Coordinates the overall interdisciplinary plan of care from admission to discharge.
• Monitors care and liaises between patient/family, healthcare personnel, and insurers.
• Evaluates patient needs and resource availability to optimize the plan and outcomes.
• Develops discharge plans with resources to meet ongoing needs and encourage adherence.
• Identifies care issues and suggests revisions to clinical pathways to improve quality of care.
• May conduct utilization reviews.
