Medical University of South Carolina is seeking a Nurse Case Manager I to oversee the plan of care across the hospital's continuum. You will drive discharge planning, resource utilization, and patient transitions to promote timely, quality care.

The role requires a nursing degree with at least one year of experience, an active SC or compact state RN license, and current BLS certification. Leadership and CM certifications are preferred to support our care transition goals.

Formación

• Bachelor of Nursing degree preferred with at least one year of experience.
• RN licensure in SC or a compact state.
• Current BLS certification required.
• One year of case management/utilization review preferred.
• Case Management certification preferred.
• BSN enrollment within two years; BSN degree within four years.

Responsabilidades

• Manage the plan of care across the care continuum to ensure efficient patient movement.
• Serve as clinical expert for discharge planning and resource utilization.
• Lead case management activities and coordinate transitions out of hospital.
