Job summary

Medical University of South Carolina seeks a Nurse Case Manager II who coordinates a comprehensive plan of care and guides discharge planning within the Case Management & Care Transitions team. Reporting to the Manager of Case Management, you will act as a clinical expert to promote safe, timely discharge and smooth patient flow through the hospital.

Requirements include a BSN, SC/compact RN licensure, and three years of RN experience; leadership and Case Management certification are preferred.

Qualifications

• Bachelor's degree in Nursing required.
• SC or compact RN licensure required.
• 3 years RN experience required; leadership experience preferred.
• Case Management certification preferred.
• BLS required.

Responsibilities

• Coordinate a plan of care to achieve clinical outcomes and promote safe, timely discharge.
• Serve as a clinical expert in discharge planning, resource utilization, and case management.
