Responsibilities

• Disposition planning and timely discharge planning.
• Identify barriers to timely disposition and coordinate with care team.
• Provide education to patient/family on care plan and resources.
• Support advance directives discussions and continuity of care.
• Collaborate with physicians and case managers to optimize care and avoid unnecessary readmissions.
