Job summary

TruHeart Home Health is seeking a compassionate PRN Registered Nurse to provide in-home nursing care in Norman, OK, including Edmond and surrounding areas. You will follow a Plan of Care, perform assessments, and collaborate with physicians to meet patient needs.

The role requires RN licensure in Oklahoma, CPR, a valid driver's license, and reliable transportation. Prior home health or hospital experience is preferred, with competency assessments completed.

Qualifications

Responsibilities

Job Summary

We are seeking a compassionate and skilled PRN Registered Nurse to join our team at TruHeart Home Health & Hospice in Norman, OK. As a PRN RN, you will provide prescribed nursing care services to patients in their homes, following a Plan of Care (POC) and according to their needs.

Responsibilities

• Conduct a physical examination of patients, including vital signs, physical assessment, mental status, appetite, and diet, etc.
• Evaluate patients, family members, and homes to determine what health teaching will be required.
• Develop and implement a written Plan of Care with patients, following physician's orders and agency guidelines.
• Perform accurate assessments, develop and implement a written Plan of Care with patients, and adapt these actions as patient needs change.
• Administer medications, treatments, and IVs according to agency policy, accepted standards of care, and home health guidelines.
• Communicate effectively with patients, families, and physicians to ensure quality patient care.
• Document patient status, patient needs, nursing interventions, and patient response.
• Conduct admission, transfer, recertification, and discharge visits for patients.
• Assist with orders, assessments, wound care, lab draws, and other tasks as assigned.

Requirements

• Education: Graduate of an accredited school of nursing for RN
• Experience: One or more years in home health agency or in hospital setting
• Training: Competency assessments must be completed
• License/Certification: Current Oklahoma RN license, CPR, driver's license, and auto insurance

Technology/Equipment Operation

• Instruments and equipment used on a regular basis: thermometer, blood pressure cuff, glucometer, PT/INR machine, PICC line, wound vac, IV skills competency will be evaluated upon hire.

Job Conditions

• Must have a driver's license and be willing to drive to patient's residence in Norman, OK, OKC, Edmond, and surrounding areas.
• Ability to communicate both verbally and in writing is required.
• Ability to access a patient's home, which may not be routinely wheelchair accessible, is required.
• Physical activities will include walking, sitting, stooping, and standing and minimal to maximum lifting of patients and the turning of patients.
• Environmental conditions: moderate noise level; inherent potential for exposure to bloodborne pathogens, moderate stress, and emotional demands.
