Job summary

TruHeart Home Health and Hospice in Tulsa, Oklahoma, is seeking a skilled and compassionate PRN Licensed Practical Nurse (LPN) to provide in-home nursing care under a Plan of Care. This is a per diem role with flexible scheduling.

The LPN will assess patients, administer medications, coordinate with RNs/MDs, and support rehabilitation and education while maintaining safety and quality of care in home settings.

Qualifications

• Current Oklahoma LPN license.
• CPR certification.
• Driver's license and auto insurance.
• One or more years of experience in home health agency or hospital setting.
• Competency assessments must be completed.
• Graduate of an accredited school of nursing for LPN.
• Ability to access patient homes, which may not be wheelchair accessible.
• Hearing, eyesight, and physical dexterity sufficient to perform patient care.

Responsibilities

• Provide comprehensive health and psychological evaluations, monitoring of patient condition, health promotion, and prevention coordination of services, and direct nursing care.
• Demonstrate knowledge and competency in performing baseline nursing skills as outlined in agency policies and clinical policies.
• Implement nursing care to patients in the home setting, working under the direct supervision of an RN/MD.
• Administer medications and treatments according to Agency policy, accepted standards of care, and home care guidelines.
• Assist with rehabilitation according to the medical care plan, encouraging patients to help themselves using their own capabilities.
• Identify and determine priority of patient needs/problems, anticipate emergencies, and report abnormalities to the RN/MD.

As a PRN LPN, you will provide prescribed nursing care services to ill, injured, convalescent, and disabled patients in their homes, following a Plan of Care (POC) according to the patient's needs.
