Job summary

TruHeart Home Health provides compassionate in-home nursing care, seeking a PRN RN to join our Bartlesville/Okmulgee area team. You will assess patients, develop plans of care, and deliver nursing interventions according to physician orders and agency guidelines.

You will monitor vital signs, perform wound care and IVs as needed, document accurately, and communicate with patients and families to ensure high-quality care. A focus on continual learning and adherence to policies is essential.

Qualifications

  • Graduate of an accredited school of nursing.
  • One or more years of experience in home health or hospital setting.
  • Competency assessments must be completed.
  • Current Oklahoma RN license, CPR, driver's license, and auto insurance.
  • Ability to interact with a computer and iPad for charting and documentation.

Responsibilities

  • Conduct physical examinations and assessments of patients, including vital signs and status.
  • Develop and implement written Plans of Care with patients following physician orders.
  • Perform nursing interventions and patient education per the Plan of Care.
  • Demonstrate baseline nursing skills including wound care, lab draws, IVs, and more.
  • Communicate effectively with patients, families, and healthcare teams.
  • Document patient status accurately and concisely.
  • Evaluate the effectiveness of nursing services and identify areas for improvement.

Job Summary:

We are seeking a dedicated and compassionate PRN RN to join our team at TruHeart Home Health - Bartlesville / Okmulgee area. As a PRN RN, you will provide prescribed nursing care services to patients in their homes, following a Plan of Care (POC) according to their needs. Responsibilities:
  • Conduct physical examinations and assessments of patients, including vital signs, physical status, mental status, and diet
  • Develop and implement written Plans of Care with patients following physician's orders and according to agency guidelines
  • Perform and document nursing interventions, including patient education, according to the Plan of Care
  • Demonstrate knowledge and competency in performing baseline nursing skills, including wound care, lab draws, IVs, and more
  • Communicate effectively with patients, families, and healthcare teams
  • Document patient status accurately and concisely
  • Evaluate the effectiveness of nursing services and identify areas for improvement
Additional Responsibilities:
  • Conform to all applicable Agency policies and procedures
  • Perform related duties as assigned or requested
  • Continuously strive to improve nursing care through in-service education, formal education, and individual research
  • Assume accountability for reporting incidents and complaints according to Agency policy
Requirements:
  • Graduate of an accredited school of nursing
  • One or more years of experience in home health or hospital setting
  • Competency assessments must be completed
  • Current Oklahoma RN license, CPR, driver's license, and auto insurance
  • Ability to interact with a computer and iPad for charting and documentation
Work Environment:
  • Moderate noise level
  • Inherent potential for exposure to bloodborne pathogens
  • Moderate stress and emotional demands
  • Ability to work in a variety of environments, including homes with varying conditions
Physical Requirements:
  • Sitting (3-6 hours daily)
  • Standing (occasionally 3 hours or less daily)
  • Walking (occasionally 3 hours or less daily)
  • Twisting, bending, squatting, kneeling, crawling, and climbing (occasionally 3 hours or less daily)
  • Wrist deviation (side to side) and hand/wrist repetitions (up and down) (occasionally 3 hours or less daily)
