Job Summary:

We are seeking a Per Diem Licensed Practical Nurse (LPN) to join our team at TruHeart Home Health - Norman. As a PRN LPN, you will provide compassionate and high-quality nursing care to patients in the comfort of their own homes. Essential Functions:

• Provide prescribed nursing care services to patients in the home care setting, following a Plan of Care (POC) and according to patient needs

• Monitor patient condition, report abnormalities, and make necessary adjustments to care plan

• Assist patients with daily living activities, such as bathing, dressing, and grooming

• Administer medications and treatments as prescribed by physicians

• Maintain accurate and concise records of patient care and status

• Collaborate with other healthcare professionals to ensure comprehensive care

• Participate in on-call rotation as required

• Perform selected skills actions, such as wound care and IV maintenance

• Identify and address patient needs and problems, and report concerns to supervising nurse

• Maintain a professional and respectful attitude, and promote teamwork and harmony with patients, family, and staff

Responsibilities:

• Demonstrate knowledge and competency in performing baseline nursing skills and procedures

• Implement nursing care plans and make necessary adjustments

• Participate in patient care conferences and team meetings

• Maintain a safe and efficient work environment

• Follow all agency policies and procedures
