Job summary

Join UnitedHealth Group as a PRN Registered Nurse in home health. You will coordinate and oversee patients' home care, collaborate with multidisciplinary care teams, and educate families on patient needs.

Expect thorough assessments, medication management, and timely documentation to support quality care. Responsibilities include supervising LPNs and HHAs, communicating progress to managers and physicians, and contributing to care plans tailored to each patient.

Qualifications

• Current RN license for the state(s) you work in
• Current CPR certification
• 1+ years of clinical RN experience (or approved by VP of clinical)
• Valid driver's license and reliable transportation

Responsibilities

• Coordinate and oversee patients' home care
• Collaborate with care teams to implement care plans
• Educate families on patient condition and needs
• Perform head-to-toe physical assessments
• Monitor vital signs and manage medications
• Administer injections, IV therapy or tube feeds
• Administer and maintain catheters
• Complete documentation timely and accurately
• Supervise LPNs and HHAs
• Communicate patient progress to the clinical manager and care team
• Take on other duties as assigned
