Job summary

UnitedHealth Group is seeking a compassionate Registered Nurse to join our home health team. You will provide in-home nursing care, coordinate plans of care with physicians and families, and ensure compliance with state regulations.

Part-time and PRN opportunities are available, with responsibilities spanning clinical supervision, patient assessments (OASIS), documentation, and collaboration with a multidisciplinary team to support quality, safety, and continuity of care in patients' homes.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and access to reliable transportation.
• CPR Certification or ability to obtain within 90 days.
• Ability to work with diverse patient populations.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with physicians and team.
• Supervises home health aides and licensed nurses, assigns tasks per regulations.
• Performs supervisory visits and ensures compliance with policies.
• Completes assessments (OASIS) and medication reconciliation, documents visits.
• Maintains high-quality clinical services coordinating with care team.
• Communicates with physicians, patients, and families to ensure quality care.
• Participates in on-call and weekend rotations as needed.
