Qualifikationen

• Current and unrestricted RN licensure in state of practice.
• Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.

Aufgaben

• Develops, updates, and communicates the plan of care in collaboration with the physician and health care team.
• Supervises home health aides and licensed nurses, providing instruction and task assignment per regulations.
• Performs comprehensive nursing assessments (OASIS), medication reconciliation, and timely documentation.
• Ensures infection control standards are followed and maintains quality patient care across home visits.
