Job summary

UnitedHealth Group seeks an experienced Registered Nurse for Home Health to deliver nursing care in patients' homes as prescribed by physicians and in line with agency policies. You will plan and coordinate care with the broader health care team to ensure continuity and quality outcomes.

The role requires an active RN license in the practice state, with CPR certification preferred and home health experience. You may participate in on-call rotations and contribute to a patient-centered care plan.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current CPR certification a plus or ability to obtain within 90 days.
• 1+ year of Home Health experience preferred.

Responsibilities

• Initiates, develops and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises home health aides and licensed nurses, providing instruction and task assignment according to regulations.
• Provides supervisory visits and ensures quality care and patient safety.
• Performs comprehensive assessments (OASIS) and medication reconciliation.
• Documents visits per policy and payer requirements, and coordinates with care team.
• Maintains communication with physicians, patients and families to ensure continuity of care.
