Job summary

The ideal candidate graduated from an accredited PTA program, has experience in skilled nursing or rehab settings, and is proficient in documentation and teamwork.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Strong documentation and communication skills.

Responsibilities

• Provide physical therapy treatments as directed by the supervising Physical Therapist.
• Implement individualized treatment plans based on physician orders and therapy goals.
• Monitor resident progress and communicate changes to the Physical Therapist.
• Document treatment sessions and progress notes accurately and timely.
• Collaborate with nursing staff to achieve positive resident outcomes.
