Job summary

Post Rehabilitation Services at The Elms Center in Milford, NH seeks a dedicated Physical Therapist (PT) for a full-time role. You will evaluate residents, design individualized plans, and deliver interventions to improve strength, balance, gait, and mobility, while coordinating with an interdisciplinary team.

Applicants must hold an active NH PT license and have completed an accredited PT program. New grads are encouraged to apply, with opportunities for growth within a diverse long-term care

Qualifications

• Active New Hampshire Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical judgment, communication, and organizational skills.
• New graduate Physical Therapists are encouraged to apply.
• Skilled nursing or post-acute care experience preferred, but not required.

Responsibilities

• Perform physical therapy evaluations and establish individualized plans of care
• Provide skilled interventions addressing strength, balance, gait, transfers, endurance, and mobility
• Develop functional rehabilitation goals and track resident outcomes
• Modify treatment approaches based on clinical findings and resident progress
• Educate residents and caregivers on exercises, mobility strategies, and safety
• Maintain thorough and timely clinical documentation
• Coordinate care with PTAs, OT, SLP, nursing, and other healthcare professionals
