Job summary

Premier Healthcare, LLC is seeking a compassionate Physical Therapist Assistant (PTA) to join our in-house rehabilitation team in Leroy, NY. This PRN role offers flexibility for motivated individuals who want to help residents improve mobility, strength, and independence under PT supervision.

New graduates are welcome. You will provide resident-centered care, document progress, and collaborate with a multidisciplinary team to support meaningful gains and a higher quality of life for residents.

Qualifications

• Active Physical Therapist Assistant license in STATE, in good standing.
• Skilled nursing or long-term care experience preferred.
• Knowledge of Net Health Electronic Documentation System preferred.
• Strong clinical, communication, and organizational skills.

Responsibilities

• Provide skilled physical therapy treatments under the supervision of a Physical Therapist.
• Implement individualized plans of care to reduce pain and improve function.
• Monitor and document resident progress, including strength, mobility, and motor performance.
• Communicate changes in resident condition to the Physical Therapist and care team.
• Educate residents, families, and caregivers.
• Collaborate with the interdisciplinary care team.
• Maintain accurate, timely, and compliant documentation.
• Follow all professional, legal, and regulatory standards.
