Job summary

Afton Oaks Nursing & Rehabilitation Center in Houston is seeking a compassionate Physical Therapy Assistant (PTA) to join our in-house rehab team on a PRN basis. Under the supervision of a Physical Therapist, you will help residents improve mobility, strength and overall quality of life with person-centered care.

Responsibilities include administering prescribed PT treatments, documenting thoroughly, and collaborating with the interdisciplinary team to adjust plans of care while communicating

Qualifications

• Texas PTA license in good standing.
• SNF/long-term care experience preferred.
• Knowledge of Net Health Electronic Documentation System preferred.
• Strong communication and teamwork skills.

Responsibilities

• Administer prescribed PT treatments to improve mobility and function.
• Provide PT services under Physician orders and PT supervision.
• Relay service requests to the Physical Therapist.
• Modify care plans under PT supervision as needed.
• Measure and record motor function, strength, and performance.
• Monitor patient responses and adjust approaches with the team.
• Educate patients, families, and caregivers.
• Ensure thorough timely documentation per policies and regs.
• Attend meetings designated by the Director of Rehab.
