Job summary

Five Points Nursing and Rehabilitation seeks a full-time Physical Therapy Assistant (PTA) to join an in-house rehab team. You will administer prescribed PT treatments to help residents improve mobility, strength, and quality of life under the supervision of a Physical Therapist.

The role emphasizes patient-centered care, accurate documentation, and collaboration with an interdisciplinary team. Texas PTA license is required, SNF/LTC experience preferred, and familiarity with Net Health is a plus.

Qualifications

• Active and valid Texas PTA license.
• In good standing with regulatory agencies.
• Strong communication and teamwork skills.
• Ability to manage patients with varying needs and personalities.

Responsibilities

• Administer prescribed physical therapy treatments.
• Provide PT services under supervision of a Physical Therapist.
• Relay information and referrals to the PT on service requests.
• Implement and modify plans of care under PT supervision.
• Measure and record motor function, strength, and muscle performance.
• Monitor patient response and adjust treatment with the team.
• Provide education to patients, families, and caregivers.
• Ensure thorough documentation per policies and regulations.
• Attend required Rehab Director meetings.
