Job summary

Community Health Network (CHN) is seeking a PRN Physical Therapist Assistant (PTA) to provide skilled home health therapy in Wayne and Stark Counties, Ohio. Travel to patient homes and surrounding areas as needed with mileage reimbursement.

The PTA will work under a licensed Physical Therapist, delivering interventions, patient education, and documentation in home settings. Must be dependable, compassionate, and able to operate independently.

Qualifications

• Home health, developmental disability, rehabilitation or related clinical experience preferred.
• Strong communication, organizational, and documentation skills.
• Ability to work independently while maintaining communication with the supervising PT.
• Valid driver's license and reliable transportation.
• Must meet driver insurability requirements.
• Must successfully complete required background checks and employment screening.

Responsibilities

• Provide skilled PTA services under the supervision of a licensed Physical Therapist and plan of care.
• Assist patients with therapeutic exercise, strengthening, balance, mobility, gait, and transfer training.
• Educate patients and caregivers on home exercise programs and safety.
• Observe and document patient response and progress.
• Communicate significant changes to the supervising PT and clinical staff.
• Maintain accurate clinical documentation for all services.
• Coordinate care with the interdisciplinary team and adhere to policies.
