Job summary

Patriot Homecare is seeking a Physical Therapy Assistant (PRN) to join our home health team serving Stark and Mahoning Counties. You will work under a Registered PT to deliver delegated patient services and participate in plan of care development.

The role requires PT supervision, note preparation, and occasional re-evaluation as needed. A two-year PTA program, Ohio PTA license, and CPR/BLS certification are required, with a competitive per diem visit rate and supportive team environment.

Qualifications

• Must graduate from an approved school offering a two-year PTA program.
• Must be licensed by the State as a Physical Therapy Assistant.
• Must have a criminal background check.
• Must have a current CPR (BLS) certification.

Responsibilities

• Under the direction of a Registered Physical Therapist (PT), provide patient services delegated by the PT.
• Under PT supervision, assist in evaluating and developing a rehabilitative plan of care and in periodic re-evaluation as required.
• Participate in the preparation of clinical and daily progress notes.
• Assume other duties deemed appropriate by the PT and as designated by the RN Case Management and Nursing Supervisor.
