Job summary

Elara Caring is seeking a dedicated Physical Therapist to join our home health team in Texas. You will evaluate patients in their homes, design individualized treatment plans, and monitor progress to promote independence and quality of life.

You will collaborate with doctors, caregivers, and the care team to ensure seamless, effective care and timely documentation. This role requires a graduate of an APTA-approved program, active PT license, and at least one year of healthcare experience.

Qualifications

• Graduate of an APTA-approved physical therapy program.
• Current, unrestricted PT license valid for the state of work (Texas).
• Minimum one (1) year of experience in a healthcare facility.
• Must possess a valid driver's license or reliable transportation to travel between patient homes.
• Ability to frequently lift, push, pull, and move up to 50 pounds.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.

What is Required?

• Minimum one (1) year of experience in a healthcare facility
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
