Job summary

Concept Rehab is seeking PRN Physical Therapists to support multiple SNF facilities in the greater Cincinnati, Ohio area. Clinicians choose preferred locations and schedules within the network, enabling work to fit personal needs while delivering high-quality rehabilitative care.

Responsibilities include evaluating residents, creating individualized plans, collaborating with the care team, supervising PTAs as required, and participating in discharge planning.

Qualifications

• Licensed Physical Therapist in Ohio.
• Ability to evaluate residents and develop individualized treatment plans.
• Strong communication and collaboration with care teams.
• Experience documenting care and billing in a timely manner.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services to improve mobility and function.
• Collaborate with physicians, nurses, families, and interdisciplinary team members.
• Supervise PTAs as required.
• Participate in care and discharge planning.
• Recommend equipment and interventions to improve outcomes.
• Complete timely documentation and billing.
• Educate patients, families, and staff on programs.
