Job summary

Concept Rehab is seeking PRN Occupational Therapists to provide services across multiple SNF locations in the Cincinnati area. You will evaluate residents, develop individualized treatment plans, and deliver skilled OT services to improve independence, safety, and quality of life.

Work locations are chosen by you based on geography and availability; Therapists are not required to work at every facility. Flexible PRN scheduling and collaboration with interdisciplinary teams ensure quality patient

Qualifications

• Bachelor's, Master's, or Doctorate in Occupational Therapy.
• Current Ohio OT license.
• Strong clinical judgment and collaboration skills.

Responsibilities

• Evaluate residents and create individualized OT treatment plans.
• Deliver OT services to improve independence, safety, and quality of life.
• Address ADLs, upper-extremity function, cognition, positioning, and mobility.
• Supervise COTAs as appropriate.
• Collaborate with physicians, nursing, families, and care teams.
• Participate in care conferences and discharge planning.
• Recommend adaptive equipment and interventions to maximize patient outcomes.
• Complete documentation and daily billing.
• Educate residents, caregivers and staff on programs.
