Concept Rehab is seeking PRN Physical Therapists to support multiple skilled nursing facilities in the Dayton, Ohio region. You will evaluate residents and deliver skilled PT services focused on improving mobility and independence.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy.
• Current Ohio Physical Therapist license.
• Strong communication and clinical decision-making skills.
• Commitment to providing compassionate, patient-centered care.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services to improve mobility, strength, function.
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members.
• Supervise Physical Therapist Assistants (PTAs) as required.
• Participate in care planning and discharge planning.
• Complete timely, accurate documentation and billing.
• Educate patients, families, and facility staff on maintenance and restorative programs.
