Job summary

Concept Rehab is seeking PRN Physical Therapists to support multiple SNF facilities in the greater Cincinnati, Ohio area. Clinicians choose preferred locations and schedules within the network, enabling work to fit personal needs while delivering high-quality rehabilitative care.

Responsibilities include evaluating residents, creating individualized plans, collaborating with the care team, supervising PTAs as required, and participating in discharge planning.

Qualifications

• Licensed Physical Therapist in Ohio.
• Ability to evaluate residents and develop individualized treatment plans.
• Strong communication and collaboration with care teams.
• Experience documenting care and billing in a timely manner.

PRN Physical Therapist (PT) | Multiple Cincinnati-Area SNF Locations

Flexible Schedule | Therapist-Owned & Operated

We are currently seeking PRN Physical Therapists (PTs) to support multiple skilled nursing and rehabilitation facilities throughout the Cincinnati, Ohio market.

Opportunities are available in:

• North Central Cincinnati/I-75 Corridor
• Mt. Airey/College Hill
• Mt. Healthy/Springfield Township
• West Side/Delhi Area
• Loveland

Candidates are not expected to work at all locations. Facility assignment is based on applicant preference, scheduling availability, and geographic convenience.

This position supports a network of skilled nursing and rehabilitation facilities throughout the greater Cincinnati area. During the interview process, our recruiting team will discuss preferred work locations and travel expectations with each candidate.

Requirements

What You'll Do

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services to improve mobility, strength, function, and quality of life
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members
• Supervise Physical Therapist Assistants (PTAs) as required
• Participate in care planning and discharge planning
• Recommend appropriate equipment and interventions to support patient outcomes
• Complete timely, accurate documentation and billing
• Educate patients, families, and facility staff on maintenance and restorative programs

Qualifications

• Bachelor's, Master's, or Doctorate Degree in Physical Therapy
• Current Physical Therapist license in the State of Ohio
• Strong communication and clinical decision-making skills
• Commitment to providing compassionate, patient-centered care
