Qualifications

Responsibilities

• Performs planned services delegated and supervised by Physical Therapist to meet patient goals.
• Assists patient with exercises and activities to improve mobility and function in home settings.
• Follows Plan of Care and agency policies under supervision of PT; updates progress with PT.
• Attends case conferences and team meetings; participates in on-call rotation as required.
• Maintains patient rights, dignity and confidentiality; completes timely documentation.
