Job summary

Paradigm Home Health Companies seeks a PRN Physical Therapist Assistant in Granbury, TX to implement the plan of care under guidance of a physical therapist and to coordinate with the care team. You will educate patients and families on home safety and medical equipment, and motivate patients to reach their goals.

Role requires independence, strong communication, and comfort with technology and EMR systems.

Qualifications

• Associate Degree in Physical Therapy.
• Current Certification in BSL/CPR.
• Home Health Experience.
• Valid Texas Driver's License and Automobile Insurance.
• Knowledge of WellSky EMR software is a plus.
