Graceworks Lutheran Services is a not-for-profit organization serving over 2000 older adults and adults with disabilities. Bethany Village in Centerville, OH provides top‑of‑the‑line care on 100 acres. A PRN Physical Therapist is needed for Saturday coverage, 8:30am–5:00pm, at $52/hr.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited college.
• Minimum two years of physical therapy experience in a long-term health care setting.
• Current Ohio Physical Therapy License.

Responsibilities

• Administers physical therapy in accordance with standards and guidelines to ensure quality care.
• Interacts with health professionals, residents, families, and support groups to maximize rehabilitation.
• Evaluates residents with muscle, neurological, gait, and functional tests.
• Develops treatment programs and conducts ongoing assessments and modification as needed.
• Maintains records including evaluations, progress notes, discharge summaries, and attendance.
