Job summary

The Grove at Valhalla Rehabilitation and Nursing Center is seeking a Licensed Physical Therapist for weekend PRN coverage in a busy 160-bed facility. You will evaluate referred patients, design and implement rehab plans, supervise assistants, and document progress while educating families.

The role emphasizes patient-centered care within a multidisciplinary team. New graduates are welcome to apply, with a focus on geriatric and long-term care settings.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid NY state license and registration.
• Experience with long-term care and Geratric populations preferred.
• Excellent documentation skills and patient service.
• Ability to collaborate with multidisciplinary teams.
• Punctuality and professional conduct.

Responsibilities

• Assess referred patients and provide direct patient care to determine rehabilitation needs.
• Develop and administer treatment plans with goals aligned to patient capacity.
• Document patient condition and educate patients and families.
• Oversee Physical Therapy Assistants and coordinate with other disciplines.
• Maintain confidentiality and adhere to HIPAA.
