Job summary

AdvantageCare Rehabilitation is seeking a PRN Physical Therapist (PT) to join the Grand Rapids Care Center rehab team in Grand Rapids, OH. The role offers flexible PRN hours, with opportunities across weekday and weekend shifts, enabling you to balance other commitments while contributing to geriatric rehabilitation.

You will evaluate and treat adults in a skilled nursing setting, collaborate with a multidisciplinary team, and drive patient-centered plans of care in a supportive environment.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current or eligible Ohio Physical Therapist license.
• Strong clinical judgment and communication skills.
• Ability to work collaboratively with PTAs and the interdisciplinary team.
• Geriatric or skilled nursing rehabilitation experience is a plus.
• Patient-first approach to care.

Responsibilities

• Evaluate and treat patients with mobility and functional needs.
• Develop individualized plans of care based on patient goals.
• Address strength, balance, gait, transfers, endurance and functional mobility.
• Provide therapeutic exercise, neuromuscular re-education and mobility training.
• Assess need for assistive devices and provide training.
• Collaborate with PTAs and interdisciplinary care team.
• Complete accurate and timely clinical documentation.
• Help patients achieve greater safety, mobility and independence.
