Job summary

Antelope Valley Care Center is seeking a PRN Physical Therapist to evaluate residents, develop care plans, and promote mobility and independence. The role requires licensure in the state and entails supervising PT assistants and coordinating with physicians, staff, residents, and families.

The incumbent will maintain timely documentation, provide in-services, and support quality rehabilitation program development.

Qualifications

• Bachelor’s degree in Physical Therapy; graduates with Masters/Doctorate preferred.
• Proficient in Net Health and Point Click Care software.
• Licensed as a Physical Therapist in the state; maintain license in good standing.

Responsibilities

• Evaluate residents and determine PT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and facilities staff involved in resident care.
• Communicate progress and discharge plans with physicians, families, and team.
• Complete timely documentation in accordance with laws and payers.
• Provide in-services and training for facility staff as needed.
