Job summary

Vantage Care Management seeks a Physical Therapist (PRN) to plan, develop, organize, implement and direct the facility's physical therapy services, ensuring high-quality resident care in accordance with federal and state standards.

The PT will evaluate patients, devise customized rehabilitation plans and coordinate with physicians and staff to restore function and maximize quality of life for residents in the skilled nursing setting.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct physical therapy services and programs.
• Assess patients and determine appropriate PT care.
• Maintain documentation, records, and reports for PT services.
• Communicate effectively about PT programs with staff and residents.
• Supervise and direct physical therapy assistants and aides as needed.
• Administer prescribed physical therapy treatments and modalities.
• Reassess patients and adjust treatment plans to maximize function and independence.
• Contribute to discharge planning and resident care plans.
