Job summary

Antelope Valley Care Center is seeking a Staff Physical Therapist to evaluate and treat residents, develop individualized plans, and coordinate care with physicians, families, and the health team. You will supervise PT assistants, conduct in-services, ensure compliant documentation, and promote safety and fall prevention.

A Bachelor's in Physical Therapy is required, with preference for Masters or Doctorate; licensure in the state and CPR if required.

Qualifications

• Bachelor's degree in Physical Therapy required.
• State licensure as a Physical Therapist; CPR certification if required.
• Proficiency with Net Health and Point Click Care for documentation.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and support staff in resident care.
• Communicate with physicians, nursing staff, and families regarding progress and discharge planning.
• Maintain timely, accurate documentation per facility and regulatory requirements.
