Job summary

Antelope Valley SNF Healthcare, LLC is seeking a Staff Physical Therapist to evaluate residents, develop treatment plans, and coordinate with physicians, families, and the health team. Provide instruction to staff and supervise PT assistants; ensure documentation and compliance.

Responsibilities include improving mobility, balance, and independence; training Restorative Aides; promoting safety and fall prevention; and participating in care conferences.

Qualifications

• Bachelor’s degree in Physical Therapy and active licensure in the state.
• Masters/Doctorate in Physical Therapy preferred.
• Experience documenting care in medical records and using standard PT software.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and ensure compliant documentation.
• Educate residents and families on treatment goals and home programs.
• Maintain infection control and safety in the treatment area.
