Job summary

Human Touch Home Health is seeking a PRN Physical Therapist to serve patients in Remington and nearby communities. You will perform in-home evaluations and develop individualized plans of care to improve mobility and independence.

The role offers flexible scheduling and mileage reimbursement, with a focus on one-on-one patient care and collaborative clinical leadership to support growth.

Qualifications

• Active Virginia Physical Therapist License Required.
• Graduate of an accredited Physical Therapy program.
• Valid Driver's License and reliable transportation.
• Strong clinical assessment and communication skills.
• Ability to work independently in patients' homes.
• Home Health Experience preferred, but Not Required.
• WellSky Experience is a Plus.

Responsibilities

• Complete physical therapy evaluations in patients' homes.
• Develop individualized plans of care.
• Provide therapeutic exercise, gait, balance, transfer, and mobility training.
• Help patients improve strength and functional independence.
• Assess home safety and fall risks.
• Educate patients and caregivers.
• Monitor progress and adjust treatment plans.
• Coordinate care with physicians, nurses, therapists, and other team members.
• Complete accurate and timely documentation using WellSky.
• Participate in reassessments and discharge planning.
