Job summary

Human Touch Home Health seeks a PRN Physical Therapist to provide in-home therapy in Remington, Opal, and nearby communities. Build your caseload around your availability while delivering one-on-one care in patients' homes.

You’ll complete evaluations, plan care, and work with a clinical team; mileage is reimbursed and growth opportunities exist within a supportive leadership structure.

Qualifications

• Active Virginia PT license required.
• Graduate of an accredited physical therapy program.
• Valid driver’s license and reliable transportation.
• Strong clinical assessment and communication skills.

Responsibilities

• Complete physical therapy evaluations in patients' homes.
• Develop individualized plans of care.
• Provide therapeutic exercise, gait, balance, and mobility training.
• Help patients improve strength and functional independence.
• Assess home safety and fall risks.
• Educate patients and caregivers.
• Monitor progress and adjust treatment plans.
• Coordinate care with physicians, nurses, therapists, and other team members.
• Document accurately in WellSky and participate in reassessments
