Qualifications

Responsibilities

• Perform in-home physical therapy evaluations and develop individualized plans of care.
• Provide therapeutic exercise, gait, balance, transfer, and mobility training.
• Assess home safety and fall risks; educate patients and caregivers.

Human Touch Home Health is seeking a PRN Physical Therapist (PT) to serve patients in Remington, Opal, and nearby communities.

What You'll Do

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Help patients improve strength and functional independence
• Assess home safety and fall risks
• Educate patients and caregivers
• Monitor progress and adjust treatment plans
• Coordinate care with physicians, nurses, therapists, and other team members
• Complete accurate and timely documentation using WellSky
• Participate in reassessments and discharge planning

What We're Looking For

• Active Virginia Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Valid Driver's License and reliable transportation
• Strong clinical assessment and communication skills
• Ability to work independently in patients' homes
• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus
