Job summary

Human Touch Home Health seeks a compassionate Home Health Physical Therapist to serve patients in Richmond and surrounding communities. The role emphasizes building caseloads that fit your availability and delivering one-on-one, in-home care.

Ideal candidates hold a Virginia PT license and possess strong assessment, documentation, and communication skills. PRN scheduling provides flexibility, with mileage reimbursement and clinical leadership support.

Qualifications

• Active Virginia Physical Therapist License required.
• Strong clinical assessment and documentation skills expected.
• Excellent communication and organizational abilities needed.
• Home health experience preferred but not required.
• Experience with OASIS is a plus.

Responsibilities

• Complete evaluations, reassessments, and discharge visits.
• Develop individualized plans of care based on patient goals.
• Provide skilled physical therapy services in patients' homes.
• Educate patients and caregivers on mobility, exercises, fall prevention, and home safety.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians and interdisciplinary team members.
• Complete timely and accurate clinical documentation.
