Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team in a SNF setting. You will evaluate and treat patients following an individualized plan of care, collaborating with facility staff to ensure excellent patient outcomes.

Responsibilities include evaluating new patients, developing plans of care with measurable goals, supervising assistants, and completing timely documentation, including evaluations and progress notes.

Qualifications

• Candidate must be a licensed Physical Therapist in the United States (Illinois) or eligible for licensure.
• Experience providing care in SNF settings is preferred.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re‑evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re‑evaluations discharges and daily progress notes.
