Job summary

Enhance Rehabilitation in Kenosha, Wisconsin is seeking qualified Physical Therapists to join our growing team. In this SNF setting you will evaluate and treat patients following an individualized plan of care, with the aim of restoring function and promoting independence.

You will collaborate with facility staff, supervise Physical Therapist Assistants, document evaluations and progress notes, and communicate with patients and families about progress and outcomes.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Treating patients, addressing concerns and educating them on injury, progress and outcomes.
• Supervising Physical Therapist Assistants and guiding their daily activities.
• Re-evaluating patient progress, revising plans of care and resetting goals.
• Completing timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Collaborating with facility staff to ensure excellent patient care and service.
