Job summary

CommuniCare Rehab is seeking a PRN Physical Therapist for Kokomo Healthcare Center in Kokomo, IN. The role involves evaluating patient charts, creating and supervising treatment plans, and leading interdisciplinary team discussions to optimize resident care.

The position offers flexibility to work across multiple centers, dedicated to delivering high-quality, person-centered care and improving residents’ quality of life.

Qualifications

• Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
