Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a PRN opening for a Physical Therapist (PT) at Greenwood Healthcare Center, Greenwood, IN. This role supports our in-house rehab team and offers opportunities to work across multiple centers to meet residents' needs.

The PT will evaluate, treat, educate on care plans, and collaborate with the interdisciplinary team to ensure high-quality, resident-centered care.

Qualifications

• Active PT license in Indiana or pending license.
• Bachelor's/Master's/Doctorate in Physical Therapy from an accredited program.

Responsibilities

• Evaluate, review, and screen patients and develop a treatment plan.
• Deliver therapy directly and supervise PTA-related activities.
• Lead interdisciplinary meetings and provide staff/family education.

Qualifications

A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program. Hold a valid or pending State Physical Therapy License

Job Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
