Job summary

CommuniCare Rehab in Greenfield, IN seeks a PRN Physical Therapist to serve residents at Greenfield Healthcare Center. Our therapists are in-house team members, delivering therapy to improve quality of life and daily functioning.

The PRN position offers flexibility and potential to work at multiple centers across Indiana, collaborating with the rehab team and supervising PTAs when appropriate.

Qualifications

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, review, and screen patient charts and evaluations under physician order and develop a treatment plan.
• Implement treatment plan through direct treatment and supervise PTAs as needed.
• Attend and lead interdisciplinary meetings to relay clinical findings and make rehab recommendations.

Job Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
