Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Stafford Township, NJ. The role covers planning, organizing, developing, and directing physical therapy services to ensure high-quality resident care.

Responsibilities include evaluating residents, creating treatment plans with short- and long-term goals, documenting and billing PT services, and collaborating with the interdisciplinary team to deliver compliant, patient-focused

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to documentation and billing procedures for PT services.
• Evaluate residents to develop treatment plans with short and long-term goals.
• Conduct screenings to determine need for intervention and treatment planning.
• Communicate findings and recommendations to residents, families, and interdisciplinary team.
