Job summary

Choice Health at Home is seeking a PRN Physical Therapist to serve patients in Hernando County/Ocala area. The role involves initial exams, designing and implementing rehabilitation programs, and ongoing evaluation of patient progress in a home health setting.

Qualified candidates hold an accredited physical therapy degree, a valid Florida PT license, and CPR certification. Home health experience and a valid driver’s license are preferred.

Qualifications

• Successful completion of an accredited physical therapy program.
• Valid Physical Therapy license is required.

Responsibilities

• Assess, plan, organize, and participate in rehabilitative programs to improve mobility, relieve pain, and improve or restore physical functioning.
• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Plan and carry out individually designed programs of physical treatment to maintain or restore physical functioning.
• Evaluate effects of treatment and adjust accordingly to maximize benefit.
• Maintain documentation within company guidelines and ensure continuity of quality patient care.
