Qualifications

• Graduate of an accredited Physical Therapy program.
• Current state PT license in good standing.
• Minimum of one year of experience in physical therapy; home health or rehab preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Ability to pass background screening and compliance checks.

Responsibilities

• Perform initial and ongoing patient evaluations to assess mobility and function.
• Develop and implement individualized therapy plans."
• Provide skilled PT interventions and train patients in mobility and home safety."
• Document progress, coordinate with the care team, and ensure regulatory compliance.
