Qualifications

• Successful completion of accredited physical therapy program and state licensure exam.
• Valid Physical Therapy license
• 1 year of home health experience preferred
• CPR certification
• Valid Driver’s license
• Valid Auto Insurance
• HomeCare HomeBase experience preferred

Responsibilities

• Assess, plan, organize, and participate in rehabilitative programs to improve mobility, relieve pain and restore function.
• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Plan and carry out individually designed physical therapy programs to maintain or restore physical functioning.
• Evaluate treatment effects and adjust plans to maximize benefit.
• Apply infection control and universal precautions in patient care.
• Provide clear communication to patient, family and health care team.
• Maintain documentation in line with company guidelines.
