Job summary

Choice Health At Home is seeking a Home Health Field Registered Nurse (RN) to cover Hernando County with a PRN schedule, serving Ocala, FL. You will perform comprehensive home visits, assess clinical needs, and coordinate care with the interdisciplinary team to promote recovery and independence.

The role emphasizes patient education, ongoing assessment, care planning, and documentation. A valid RN license, CPR, and a driver's license are required, with prior home health experience preferred.

Qualifications

• Current RN license in good standing.
• CPR certification current and valid.
• Valid driver’s license and auto insurance.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patients in their homes.
• Develop, implement, and document the plan of care to ensure quality and continuity.
• Educate patients and families on self-care, nutrition, and disease management.
• Determine service scope and frequency based on acuity and needs.
• Provide clinical directions to Home Health Aide and LPN/LVN as needed.
• Maintain progress notes and communicate changes to physicians and team.
