Job summary

Choice Health at Home in Riverview, FL is seeking a PRN Physical Therapist to assess, plan, and implement rehabilitative programs that improve mobility, relieve pain, and restore function for home health patients.

You will perform initial exams, document findings, tailor individually designed treatment plans, monitor progress, and maintain compliant records while collaborating with patients, families, and health care teams.

Qualifications

• Accredited physical therapy program completed.
• Current state Physical Therapy license.
• 1 year of home health experience preferred.
• CPR certification and valid driver's license.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform initial exams and evaluate data to identify problems.
• Plan and carry out individualized physical therapy programs.
• Evaluate treatment effects and adjust care as needed.
• Maintain documentation within company guidelines.
• Communicate effectively with patients and care teams.
• Support agency philosophy and policies.
