Job summary

Palms-Home-Care is seeking a skilled Home Health Physical Therapist to join our team. You will provide individualized physical therapy services to patients in their homes, helping them improve mobility and regain independence after illness, injury, or surgery.

Responsibilities include initial evaluations, treatment planning, therapeutic exercises, and collaboration with interdisciplinary teams. A valid PT license, OASIS knowledge, and EMR experience (Kinnser) are required, along with CPR

Qualifications

• Current Physical Therapist license.
• Graduate of accredited PT program.
• 1-2 years of experience preferred in Home Health.
• Strong communication skills.
• Ability to work independently.
• Possesses and maintains current CPR certification.
• Valid driver's license and reliable transportation.
• Knowledge of OASIS.
• Experience with EMR systems such as Kinnser.
