Job summary

UnitedHealth Group's Home Health division is seeking a PRN Physical Therapist to perform assessments and evaluate patient care needs related to mobility and endurance in a home setting. You will help determine a treatment plan, deliver interventions to improve well-being, and monitor progress.

You will collaborate with the care team, document progress, and ensure adherence to state guidelines. A valid driver's license and CPR certification are preferred, with compensation reflecting per-visit

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs as part of the plan of care.
• Provides PT services within scope of practice and plan of care, coordinating with the health care team.
• Observes and reports patient responses to treatment and any changes to supervising staff.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family on plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes.
• Completes evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and frequency of care.
• Function in home settings regardless of patient demographics.
